If using OHP it must NOT be an emergency-only or family planning
only
If using Kaiser please call a OBGYN advise nurse and ask the nurse
to fax Lovejoy a referral at (503) 221-1844. Kaiser is very supportive
and faxes referrals every day except Saturdays and Sundays. Please
allow 24 hours for this fax to reach us.
Photo ID such as driver's license or school ID (particularly if
using insurance of any kind).
Cash, debit, Visa or Mastercard, any money orders accepted - Lovejoy
Surgicenter does not accept personal checks.
You are encouraged to bring ONE friend or family member with you
but PLEASE do not bring babies or children to your appointments.
Ask the receptionist for directions to nearby parks, restaurants,
children's stores, and public libraries to assist your childcare
provider with children's activities.
You MUST have a ride home on the day of your procedure unless
told otherwise. (Not w/ RU486 appointments)
